PLEASE FILL OUT THE FORM BELOW TO WITHDRAW YOUR CHILD/CHILDREN FROM SWIMMING CLASS.

This form must be submitted by the 16th of the Month to withdraw from the following month’s classes. Please fill out one form per child withdrawn.
